Paternity Proceedings

To establish the parent and child relationship, you should follow the procedures and use the forms set forth in the Paternity Proceedings Brochure.  The forms which are included on this page are interactive.   The areas with blanks may be completed on the computer you are using and either printed out, signed and filed or saved to your hard drive for future use.  You should carefully read and follow the procedures in the Paternity Proceedings Brochure.

    After the parent and child relationship has been established, if  you are in need of interim or temporary orders while you are waiting for  a final determination regarding the allocation of parental responsibilities, you should follow the procedures and use the forms set forth in the Temporary Orders Brochure.
